在上面这两句话中,用的都是with sth.+不定式的结构,为什么第一句用被动（to be taken care of）,而第二句用的是主动（to take care of）?

第一句是错的.应该是with two small children to take care of.
在with的复合结构中,表示将来的,表示为：
with + 宾语+ to do ( 无被动）.
除非下面这种情况,动作的实施者不是主语.例如：
1.With the children to be taken care of,I felt very happy.孩子有人照顾,我感到很高兴（照顾孩子的动作不是主语“我”来做的）
2.With the children to taken care of,i felt very tire.要照顾孩子,我感到很累.（照顾孩子的动作是主语“我”来做的）
综上所述,你的第一个例句：
因为是she照顾孩子,所以she下车有困难.所以我认为应该改成主动.
第一句to be taken care of修饰的是children，而children是被照顾的对象，所以此处是被动。
第二句take care of的主语是I，是I没有事情可做，所以用主动。
